At Elevate Osteopathy we are committed to keeping your personal information confidential and secure. Elevate Osteopathy’s policy is follows the Australia Privacy Principles. You have a right to know where and how your information is being managed as well as access to it at any time upon request.

Information Storage:

All personal information collected by Elevate Osteopathy is stored securely on our practice management software Cliniko. You can read information about Cliniko’s data security policies here. Should you choose to correspond via email, emails will be stored on Google Workspace severs. You can read the Google Workspace privacy notice here. Patient’s are reminded that emailing medical records can be considered a high risk form of communication with regards to potential privacy breaches.

What we collect:

It is a legal requirement that any Osteopathic Consultation must have a corresponding clinical record. In order to keep complete clinical records, Elevate Osteopathy will collect personal information about you such as:

  • Name

  • Birthdate

  • Address

  • Contact details such as phone number and email address

  • Private medical information will also be collected during a consult such as current and past medical conditions, medications and any other medical history you choose to disclose

  • Medicare number - sometimes your Medicare number is required to complete claims for Medicare for certain appointment types.

  • Correspondence with your GP and other health professionals

This data is collected from online bookings through Cliniko, patient intake forms and communications by email/phone such as email enquiries. Elevate Osteopathy does not collect any credit card, debit card or bank information.

Anonymity and Pseudonymity:

Should you choose to, you can elect to use a pseudonym and remain anonymous. Please note, this may affect Medicare Claims. Your personal information cannot be stored under a government identifier I.E. your Medicare number.

Where we store your information:

All patient data is stored on our practice management software Cliniko. Data from intake forms, online bookings and any communications from Elevate Osteopathy is collected and stored on Cliniko. Cliniko has up to date and data management policies in accordance with Australian Privacy Legislation to safely and securely store patient data on their servers. All data is encrypted. Data is stored on Australian online servers and is not sent overseas.

Why we collect your information and who it will be shared with:

Personal information is collected to maintain AHPRA compliant patient records. Your personal information is never shared with any other party without your consent. Sometimes sharing your personal information with another allied health practitioner or your GP may be required to help manage your care. We will always ask you first for permission to share any personal information with another practitioner or party. Any communication of your personal information will not occur with your consent. All data about your personal information stored on Cliniko is stored on Australian servers and will not be sent overseas.

How do I access my personal information:

You have a right to access your personal information. You can ask for a copy of all of your records to date. Retrieval of your information will not incur a fee. You can request a copy of your information by emailing admin@elevateosteopathy.com.au

How do I make a complaint or correction?

Elevate Osteopathy makes efforts to keep your personal information up to date, complete and correct. If you have need to make a correction to your personal information or a complaint about how it is stored please email admin@elevateosteopathy.com.au and we will address the issue.

Direct Marketing:

Elevate Osteopathy uses very little direct marketing e.g. Newsletter Emails, Promotional Emails. Most communications that you will receive will be booking confirmation and booking reminder emails/SMS. You can opt out of either by emailing admin@elevateosteopathy.com.au

Notification of a breach:

If there is any breach of your data, efforts to contact you with be made as soon as possible by phone or email.
